Exercise and pacing take steady work. They'll improve motion, strength and walking time for some people. Medicine may calm soreness sooner. Its effect can wear off. A brace or cane helps only while you use it. These choices work like tools in a drawer. Each tool has a different use.

What to know before a procedure
Ask how long relief lasted for people like you. You'll also need the risks and time needed afterward. Compare the procedure with exercise and medicine. Brief relief can matter if you sleep or move better. It won't be enough when the joint locks or gives way. Don't choose care merely because it sounds new.

What to compare about PRP
PRP begins with blood drawn from your arm. A clinician spins it to keep extra platelets, small blood parts used for clotting that hold healing proteins. The kept blood goes back into your joint by needle. One careful study found PRP and salt water gave similar soreness relief after one year. Other reviews found more relief with PRP. When to compare surgery alternatives
A badly worn joint may still limit you after home care. You'll want to discuss knee or hip surgery alternatives before choosing. The 2019 ACR/Arthritis Foundation guideline makes STRONG recommendations for exercise, weight loss in people with overweight or obesity, self-efficacy and self-management programs, tai chi, cane use, tibiofemoral bracing, topical NSAIDs, oral NSAIDs and intra-articular glucocorticoid injection in knee OA. OARSI 2019 designates arthritis education plus structured land-based exercise (with or without dietary weight management) as CORE treatments for knee OA and strongly recommends topical NSAIDs (Level 1A), while strongly recommending AGAINST oral and transdermal opioids (Level 5). The treatments with the strongest evidence in this condition remain the least dramatic ones.

The Cochrane review of exercise for knee osteoarthritis found high-quality evidence that land-based therapeutic exercise provides short-term benefit in pain and physical function, sustained for at least 2-6 months after the programme ends, with mild transient soreness the only reported adverse effect across 45 trials. It is the best-evidenced treatment for this condition and it costs nothing per injection.

A randomized trial in the New England Journal of Medicine compared physical therapy against intra-articular glucocorticoid injection for knee osteoarthritis and found physical therapy produced better WOMAC outcomes at one year. When a clinic offers an injection, the comparator that matters is not 'nothing' - it is a course of supervised exercise.

In a 2-year RCT, intra-articular triamcinolone given every 12 weeks for knee OA produced significantly GREATER cartilage volume loss than saline, with no significant pain benefit. The most widely used joint injection in medicine is itself associated with structural harm on repeat dosing - relevant context when a clinic frames a biologic as 'the alternative to steroid shots'.

A randomized trial of 50 completed patients (mean age 63) compared ultrasound-guided 25% dextrose prolotherapy with intra-articular normal saline for knee OA. BOTH groups improved significantly in pain, function and knee extension at every follow-up (p<0.001), and prolotherapy was neither statistically nor clinically superior to saline. Adverse events were limited to injection-site pain and bruising resolving by week 4.

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

A 2026 systematic review and meta-analysis of 28 randomized trials of intra-articular mesenchymal stem cell-based therapies in knee OA found significant improvements in several pain and function measures (delta-VAS MD -1.67; KOOS pain MD 15.37) but NO significant difference in WOMAC, KOOS quality of life or the Lequesne index, and MRI-based WORMS scores were non-significant - indicating no consistent structural benefit. Its own conclusion: these therapies serve a primarily SYMPTOM-modifying rather than STRUCTURE-modifying role, with higher frequencies of local reactions to weigh against the symptomatic benefit.
